The Hot Thing — Karlach edit makes her read as more traditionally feminine: facial shapes and textures were redrawn, wrinkles smoothed away, and makeup was added (i.e., contour, blush, enhanced lips), so the result looks noticeably different from the in-game portrait.
A chunk of the backlash frames the change as a clash with Karlach’s established persona. For them, scars, a rugged, muscular frame — signs of past fights — are part of the character’s vocabulary; stripping those away feels like flattening chapters of her life into a glossy cover.
On the other side, fans of the mod point out the obvious: this is UGC — user-generated content — so people will customize as they please. Some also call out inconsistent reactions across the fandom: tweaks that make a character less conventionally pretty often slide by without fireworks, while changes toward attractiveness trigger louder objections.
